Some persons may have been problems and are indicated as such, meaming a good vital status may not have been obtainable // Notes: "Codes: A - alive D - dead U - unknown X - a problem was encountered in ascertaining a vital status for this individual this may be a conflict, discrepancy, or other reasons Several outside sources are used in trying to ascertain the status of people. Some people become "lost to follow-up" meaning these outside sources cannot provide any information on them. These people become "unknown"." | | | ONE |

| ca8 | ICD8 code for cancer cause of death | This field contains an ICD code (8th revision) if there was a a cause of death that was a cancer. If none, the field is left blank. // Notes: ICD8 is the ICD code that that nosologist has determined to be the 'underlying' cause of death of the individual. There are basically two methods of coding - 'underlying' and 'all causes'. The decision was made early on in CER's history that we would do 'underlying'. The CA8 is an ICD code containing a cancer code if something was detected by the nosologist on the death certificate indicating a cancer was present. For example, a person may have died in a motor vehicle accident and that would have been the underlying cause of death, but an autopsy also revealed that the person had lung cancer and it was noted by the physician completing the DC. The nosologist would pick up that piece of information and note it in the CA8 field. Obviously, CA8 will not be populated on every record. The 8 on both of these variable names means the ICD code is coded using the 8th revision of the ICD code structure. | | | THREE |
| fac1 | Facility code | This is a code indicating a facility the person was known to have worked at. | icd8 | ICD8 code for underlying cause of death | This field contains a 3 character ICD code (8th revision) for the underlying cause of death as determined from the death certificate. This field is simply the first 3 charcters of the ICD8X field. If the person is alive, obviously this field is blank. // Notes: Death certificates used at CER were coded for underlying cause. This field was established to allow grouping of deaths without having to manipulate the ICD8X field. Also, it may be useful to some software that calculates SMRs. | | | THREE |
| icd8x | ICD8 code for underlying cause of death | This field contains an ICD code (8th revision) for the underlying cause of death as determined from the death certificate. If the person is alive, obviously this field is blank. // Notes: Death certificates used at CER were coded for underlying cause. | | | THREE |

| conhire | Continuous employment indicator | This flag indicates if the person was known to have a continuous employment at the facility where the 5 rem exposure occurred. This means the person had no breaks in employment from the FHIRE date and the LTERM date in this record. // Notes: Codes: Y = yes, employment was continuous N = no, employment was not continuous U = unknown if continuous | | mffconhi | TWO |
| firexyr | Year in which 5 rem exposure occurred | This field contains the year in which the person received the first 5 rem exposure at the site (as defined in FAC5RM [facrm5]). | Year | | TWO |

| tot_exp | Total lifetime recorded exposure in rem | This field contains the recorded lifetime dose for this person, in rem. This value may or may not be applicable currently depending on whether the person has left the work force, if the person was deceased, or has moved to another job where exposures are possible, but these data have not been included in this file. // Notes: This field is useful primarily in comparing this cohort to itself to see how total exposures matched or compared with others. | | | TWO |
| borx | Flag: expos. may have occurred over 2 yr | This field contains a flag indicating if there was a suspicion the exposure of 5 rem occurred over a 2 year period instead of 1 year. // Notes: "Long name: Flag indicating if exposure may have occurred over 2 years Codes: B = indicates the exposure could have occurred over a 2 year period blank = no suspicion The protocol for the study stated the person must have received an exposure of 5 rem (external) within 1 calendar year. Each participating site made this determination, and some individuals were considered "borderline", thus this flag was established to indicate these people." | | mffborx | TWO |
